Population Health Manager

Description:

CareHub is seeking an experienced Population Health Manager to support a US-based client. The ideal candidate will have strong hands-on experience with HEDIS measures, gap closure, and value-based care workflows, along with prior experience supporting US healthcare clients.

🔷 𝐊𝐞𝐲 𝐑𝐞𝐬𝐩𝐨𝐧𝐬𝐢𝐛𝐢𝐥𝐢𝐭𝐢𝐞𝐬:

-Support US healthcare clients with HEDIS quality measures

-Perform gap closure activities to improve quality and compliance outcomes

-Assist in value-based care initiatives and quality improvement workflows

-Analyze and manage healthcare data to meet client requirements

-Collaborate with internal and external stakeholders

-Handle multiple tasks efficiently in a fast-paced environment

🔷𝐑𝐞𝐪𝐮𝐢𝐫𝐞𝐦𝐞𝐧𝐭𝐬:

-Hands-on experience with HEDIS, gap closure, and value-based care

-Prior experience supporting US healthcare clients

-Fluent in English (verbal and written)
