Description:

CareHub Health is seeking a Value-Based Care Coordinator to support value-based care and population health initiatives for a U.S.-based healthcare organization. The ideal candidate will have strong hands-on experience with HEDIS measures, care gap closure, quality documentation, and prior experience supporting U.S. healthcare services to improve clinical and financial outcomes under value-based care models.

🔷 𝐑𝐞𝐪𝐮𝐢𝐫𝐞𝐦𝐞𝐧𝐭𝐬:

-Fluent in English (verbal and written)

-Mandatory prior experience supporting U.S. healthcare services

-Hands-on experience with HEDIS, care gap closure, and value-based care workflows

-Minimum 1+ year of experience in Value-Based Care (VBC), Population Health, Risk Adjustment, and/or Medical Billing (preferred)

-Strong knowledge of HEDIS Quality Measures

-Proficiency in Electronic Health Records (EHRs), Microsoft Excel, and related healthcare systems

-Excellent organizational, analytical, and problem-solving skills

-Solid understanding of healthcare regulations and compliance standards
