Care Planning Pharmacist

Description:

We are looking for a qualified and compassionate Care Planning Pharmacist to join our team. The ideal candidate will support patients with chronic conditions through clinical assessments, medication management, and care coordination.

Key Responsibilities:
Conduct assessments for depression, cognitive impairment, and medication management
Review and document patient profiles per clinical guidelines
Communicate care plans to patients via calls
Develop and maintain CCM plans (medications, goals, vitals, labs)
Coordinate with providers for scheduling and billing

Requirements:
Bachelor’s or Pharm-D degree
Experience in clinical or care planning roles preferred
Strong knowledge of chronic care management
Excellent English and communication skills
