Apax Solutions is actively hiring a Patient Support Representative for our night shift, on-site team in Johar Town, Lahore.

This role is ideal for healthcare professionals with experience in US healthcare workflows and Chronic Care Management (CCM).

Key Responsibilities:

• Handle patient calls and inquiries professionally
• Manage Chronic Care Management (CCM) follow-ups and care coordination
• Schedule appointments and support daily patient communications
• Work directly with US-based doctors and healthcare providers
• Maintain accurate documentation using EHR/EMR systems

Requirements:

• Experience in patient support or healthcare services
• CCM (Chronic Care Management) experience
• Hands-on experience with EHR/EMR software
• Experience working with US healthcare providers
• Strong communication and customer service skills
• Available for night shift and immediate joining
• Willing to work on-site in Johar Town, Lahore
• Bilingual (English & Spanish) is a plus
